SAP Assessments

Fisher Counseling offers 2 types of substance abuse assessments:

DOT/PHMSA safety sensitive and non-safety sensitive  

DOT/PHMSA safety sensitive

 

A US Department of Transportation/Substance Abuse Professional Assessment (DOT/SAP assessment) occurs when an employee in a safety sensitive position tests positive for alcohol or drugs due to a pre-employment, random, self-disclosure, refused, accident, or reasonable suspicion test. A DOT evaluation is only for employees that fall under the following federal agencies: Federal Aviation Administration (FAA), Federal Motor Carrier Safety Administration (FMCSA), Federal Railroad Administration (FRA), Federal Transit Administration (FTA), Pipeline & Hazardous Materials Safety Administration (PHMSA), United States Coast Guard (USCG)

 

The SAP assessment consists of three parts: the initial interview, an intervention, and a follow-up session. The initial interview is 2 (two) hours and will consist of a comprehensive assessment and establishing an intervention. The follow-up session is 1 (one) hour. These sessions may not last the full allotted time.

 

After the initial assessment is completed, you will be directed to the appropriate level of intervention. There is a required intervention after testing positive for alcohol or a substance of abuse. The assessment determines the level of required intervention. Per Federal regulations, a third-party, not the SAP, performs the intervention. There is a separate cost to this intervention. You will pay this fee to the provider of the intervention, not Fisher Counseling LLC.

 

The SAP service fee covers the comprehensive initial assessment, intervention recommendation, letter of recommendation to your Medical Review Officer (MRO), follow up appointment, follow-up letter to the MRO, testing schedule, storage of reports, and documentation for the Federal Motor Carrier Safety Administration Clearinghouse (if applicable).

 

All reports to the MRO are sent within 24 – 48 hours.

Alcohol and Substance Abuse Evaluation (AODA)

 

The Alcohol and other Drugs of Abuse assessment (AODA assessment) consists of an initial interview, an action plan/intervention, and if needed a follow up session.

 

This assessment is appropriate for individuals that is not in a safety sensitive position and needs an assessment for school, work, or legal reasons. This evaluation meets the requirements of the Indiana Coalition for Construction Safety (CCS). I follow the U.S. Department of Transportation (DOT) and Pipeline and Hazardous Materials Safety Administration (PHMSA) guidelines for all DOT and NON-DOT assessments.

 

The Alcohol and other Drugs of Abuse assessment (AODA assessment) consist of an initial interview, an action plan/intervention, and (if needed) a follow-up session. All reports to interested parties will occur within 48-72 hours.

 

Insurance companies require care to be medically necessary. Since AODA assessments are not medically necessary, I will not file a claim through your insurance company.

 

Total cost for the evaluation is $300 payable by cash or money order only.
